Reporting Procedure

DEATH IN CUSTODY OR AS A RESULT OF POLICE ACTION
DOOD IN AANHOUDING OF AS GEVOLG VAN POLISIE-OPTREDE

11. Upon learning of a death in custody or a death as a result of police action, the following procedures must be followed:
Sodra daar kennis geneem is van 'n dood in aanhouding of 'n dood as gevolg van polisie-optrede, moet die volgende prosedures gevolg word:
         
  1.1 Report the incident immediately by telephone to your station commissioner/duty officer and the relevant provincial office of the Independent Complaints Directorate (ICD):
Rapporteer die insident onmiddellik telefonies by u stasiekommissaris/diensoffisier en aan die betrokke provinsiale kantoor van die Onafhanklike Klagtedirektoraat (OKD):

  1.2 The station commissioner must complete Form A (notification) and fax it within 12 hours to the following offices:
Die stasiekommissaris moet Vorm A (kennisgewing) voltooi en dit binne 12 ure aan die volgende kantore faks:

  1.3 If so requested, the station commissioner must complete Form B (progress report) and fax it to the four numbers given in paragraph 2 above.
Indien so versoek, moet die stasiekommissaris Vorm B (vorderingsverslag) voltooi en dit na die vier nommers aangegee in paragraaf 1.2 faks.
